This page demonstrates a simple html form template rendered in the server

You can see it changes over time so it must have a persistent storage mechanism on the backend (it does).

The code is found HERE. The survey is served via a GET and then it is designed to fire off a POST request to the same URL when a button is pushed...the resto of the code handles the POST request.

So Cool! This is code from last year...and it is still relevant since the Maple Leafs and The Boston Bruins are yet again playing one another in the first round of the NHL playoffs! Awesome
